Constituent Services/Casework: If you are looking for help with healthcare, Medicaid, housing issues, unemployment assistance, foreclosure, state-owned land or roads, disability issues or interacting with state agecies, Representative Costello can help. Health Care Related Casework: Please note that due to federal privacy laws wtih regard to personal health care information, individuals are usually required to complete a Permission to Share Information Form. This form allows the Representative's staff to investigate health-related matters on your behalf. To expedite your inquiry, please print and fill the Form out completely and then fax it to our offices: 617-722-2846.

Special Recognitions: Representative Costello frequently sends official House citations to constituents to acknowledge special occasions, including birthdays, anniversaries, Eagle Scout Awards, and other special requests. To request a citation from Mike please use the email address below or call the Boston Office.

State House Visits and Tours: If you are coming to the State House, please let the Representative know, or stop by our office in Room 167. We are happy to arrange State House tours for large groups. Individuals may take the free State House tour, which leaves every half hour from Doric Hall.
